BECOME A CERTIFIED PROVIDER

If you, or your agency, would like to apply to become a Provider of Waiver and Non-Waiver Services, you must make an application to the Ohio Department of Developmental Disabilities (DODD).

Please note that DODD is the only certifying entity for all Agency and Non-Agency (Independent) provider applicants of Supported Living, Individual Options (IO), Level I (LV1) and Self-Empowered Life Funding (SELF) Waivers. The Allen County Board of DD can merely provide support to applicants for this process, but has no authority to certify Providers.
